We accept UNITED HEALTHCARE only. If you are not a recipient of United Healthcare insurance coverage then you are expected to satisfy the fees out of pocket based on the fee argeement that you sign during your first visit.
We do provide a Sliding Fee Scale to patients in need of such services. Proof must be provided at the time of service.
Payments are due at the time of service. If you are unable to make full payment, you are expected to have satisfied the outstanding balance before or during your next visit in order to recieve treatment. If a patient refuses to pay, the account will be sent to an approved collection agency then generating an additional fee after three attempts to collect payment.
If a patient has an outstanding balance at the time of discharge, you will be given three reminders during a total of 90 days before the account will be sent to an approved collection agency then generating an additional fee.
"No Show" Fees need to be satisfied at the time of the next appointment in order to prevent suspension of services.
Federal and State law and regulations protect the confidentiality of patient records kept by this corporation. Violation of Federal and State confidentiality law and regulations is a crime and can be reported to appropriate authorities. Generally Global Healthcare Systems, Inc can not disclose any information identifying you unless: you sign a waiver, the disclosure is court ordered, or a staff member has reason to believe that a child has been subjectes to physical abuse or neglect.
Global Healthcare Systems, Inc requires that you give at least 24 hours notice that you will be unable to make an appointment. A phone or email notification is an acceptable way of communication. The fee for missing a appointment will be half of your appointment fee. Please remember to call in even if it is less than 24 hours from your appointment time so that we may be able to offer that time slot to someone else in need.
